MAIB reports on Svitzer Moira fatality

The MAIB has produced its report into the fatal accident onboard Svitzer Moira, which occurred in Royal Portbury Dock, Bristol, 29 December 2015. There were no witnesses, but the report concluded that the vessel’s engineer, who was assisting with mooring alongside another tug, had moved outside the Moira’s bulwark, but had then tripped and fallen forward between the two hull as they came together. The report stated that the company’s safe systems of work for mooring and barge handling (alongside) were adequate and had the appropriate control measures contained in them. However, the casualty had not been wearing the necessary PPE at the time of the accident and overall there had been a divergence between company instructions and practices onboard.

Svitzer has issued a safety alert to draw attention to these and is reviewing safe systems of work, risk assessments and training provided for mooring. They also implementing behavioural safety training to reemphasise to crews the consequences of not following safety procedures.
